Welkom bij de informatiepagina van bereidingsapotheek Infinity Pharma. Hier vindt u de benodigde informatie over onze producten en veilig medicatiegebruik. Duurzaamheid staat hoog op onze agenda en daarom willen we het gebruik van papier beperken. Daarom maken we zoveel mogelijk informatie en formulieren digitaal voor u beschikbaar.

Sinds kort hebben wij onze informatievoorziening gewijzigd. Lees aub hier hoe en waar u de informatie kunt vinden die voor u van toepassing is.
Bijsluiters
Bijsluiters hebben we vanaf nu digitaal beschikbaar, u ontvangt deze niet meer separaat met de verzending. Als u verder scrolt kunt u de bijsluiter(s) vinden die u nodig heeft. Op deze wijze heeft de benodigde informatie altijd tot uw beschikking. Mocht u een specifiek product missen in het overzicht, neemt u dan contact met u op.
Herhaalrecept tegoeden
Het is niet meer nodig om een papieren herhaalrecept per post naar ons te sturen, om deze reden sturen wij geen herhaalrecepten meer mee met de medicatie. Wij hebben uw herhaalrecept in ons systeem genoteerd. Afroep van een herhaalrecept kunt u doen door een bericht te sturen naar assistent@infinitypharma.com. Wij wijzen u erop dat het recept van uw arts maximaal 1 jaar geldig is.
Factuur
Als het voor u voorgeschreven middel niet wordt vergoed, ontvangt u van ons een factuur. Deze sturen we niet (meer) per post met de medicatie mee maar per e-mail naar het bij ons bekende e-mailadres. Als u uw factuur liever op een ander e-mailadres ontvangt, verzoeken wu u dit aan ons door te geven.

Medicine Information Leaflets
We have leaflets available in digitalform, we no longer send these as a separate form in the delivery package. Further on this page (scroll down) you will find the leaflet you need. If you are missing a leaflet here, please contact us.
Repeatable prescriptions
It is no longer necessary to send us a paper repeat prescription by post, for this reason we no longer send repeat prescriptions together with your medication in the parcel. We have administrated your repeat prescription in our system. You can request a repeat prescription by sending a message to assistant@infinitypharma.com. Please not that your doctor's prescription is valid for a maximum of 1 year.
Invoice
If the medicine prescribed for you is not reimbursed, you will receive an invoice from us. We no longer send paper invoices by post with the medication, but by e-mail to the e-mail address known to us. If you prefer to receive your invoice at a different email address, please let us know.
